hdf5toascii_slicer should output variable names in headers

Description

CarpetIOASCII currently puts something like

# data columns: 13:rho

into its headers. hdf5toascii_slicer does not. It would be useful if the relavant code was copied into the slicer (from outputslice.cc). Particularly useful if one translates a checkpoint file to ASCII to inspect values in detail.

It should also handle output of multiple variables properly.
