CarpetIOASCII should not write column names info each iteration

Currently, CarpetIOASCII writes the column names header each iteration, leading to bloated output - easily double the size of what should suffice. Once after file creation would be much better.

    CarpetIOASCII offers the "compact" format (CarpetIOASCII::compact_format) which reduces the amount of header material considerably.

    It does. Maybe we could make it the default. And even then the question remains whether it couldn't just write the headers once, or once in a while.
